opportunity for improvement (OFI) within your organization or practice setting
(there is an opportunity to prevent elopements/missing residents in my memory
care facility). Utilizing the Action Plan template (see attached),
identify multiple action items per category that follow SMART planning.
Attach your completed
Action Plan as an appendix to a two-page paper that describes your process and
provides supporting evidence behind your actions as they pertain to the
following key elements:

The steps for assessing the current state of the organization
·
The gap or OFI, along with the data collected to benchmark the
gap
·
An explanation of how the gap or OFI has developed
·
Identify preventable and non-preventable aspects of the issue
·
Implementation process for action items
·
The steps for assessing the effectiveness of the change and the
expectation for improving the gap or OFI
·
Strategies for supporting sustainability of the change
Your paper must follow
APA Style throughout and include at least three scholarly sources.
