1.Go to the Healthy People 2030 website link https://health.gov/healthypeople/objectives-
and-data/leading-health-indicators
2. Select one of the Leading Health Indicators listed that interest you to complete your Discussion Board post.
3. Write an initial post using the information from Healthy People 2030 and the applicable
stages of the nursing process that has four paragraphs (Use the four headings for each
paragraphs: Introduction, Assessment, Implementation, Evaluation) and submit to the
Discussion Board site named: Evidence-Based Practice: Leading Health Indicators/ Healthy
People 2030
a. Paragraph one: Introduction: Describe the Health Indicator chosen, and discuss
why you chose and were interested in this Health Indicator.
b. Paragraph two: Assessment: Review the Life Stage for this Leading Health
Indicator (who is the most vulnerable or affected group). Preview and present the
Determinant Information from the Leading Health Indicator you chose. Include any
Statistical Data that is relevant.
c. Paragraph three: Implementation: Choose two Evidence-based resources, and
use them to describe what response or actions nurses could use to participate in to
promote Evidence-Based Practice and positive outcomes re: this Leading Health
Indicator (can include programs or teaching that is in place or should be in place).
d. Paragraph four: Evaluation: What positive outcomes are expected from interventions or have been shown to work as indicated on the Healthy People 2030
website.
