The handwritten assessment may be scanned for submission.
After you submit this required assignment, print out a copy of it, and place it in your 3-prong folder.
Because of social distancing, and school closures with coronavirus (COVID-19), some of these assessments might not be feasible for you to use. So, you may substitute another Anecdotal Record, Running Record, or Work Sample Checklist. You may not reuse the form from the initial portfolio entry. The information on the form, your portfolio entry, and your photograph of the child must be new.
