Alternative to Blow-by nebulized Treatments
I believe we all have come across situations where we ask ourselves “why are we doing it this way”, “why can’t we just improve on this”. My interest revolves neonatal and pediatric intensive care, and a familiar task is trying to administer a nebulize breathing treatment via jet nebulizer method to uncooperative newborn and pediatric patients. Community standards have been to administer these treatments via blow-by. Droplet size is already inconstant with jet nebulizer and using the blow-by method, it easy why many think it’s just a waste of time and medication. Studies suggest using a mask and keeping it within a certain distance from the patients face but it is also subjective to the type of equipment or compression used. The reality is that the studies did not take into consideration real life situations of a screaming crying child, a child moving its head every which way trying to avoid the mist from the nebulizer treatment but was only a reflection of filters placed on mannequins to measure the deposition of the medication.
Effective nebulizer treatment methods that have been in use via varies equipment involve Aerogen nebulizers. Aerogen nebulizers are attached to heater chambers or in equipment circuits, example HHNC and ventilators. Aergen studies outcomes state up to six times more deposition of medication compared to jet nebulizer. Aergen nebulizers do not require pneumatic or compression to provide nebulization of medication.
Alternative plan for nebulized treatment using Arogen nebulizer via nasal canula.
Adapter using oxygen tubing between flow meter and nasal cannula was used
Color dye was placed in Aerogen chamber
Nasal cannula was placed at 1”,2”,3”,4”and 5” away from white filter
Nasal cannula flow was unchanged
Aerogen nebulizer was powered
Results concluded, using the Aergen adapter to the nasal cannula provided an even amount of color dye to the filter samples. Proving that medication was effectively nebulized through the nasal cannula tubing and prongs.
Plan using the Aerogen nebulizer adapter to nasal cannula can improve medication deposition compared to jet nebulizer via blowby, example deposition particles are consistent, giving nebulized treatment via in line will lessen any stress to the patient and family. Improving patient from being uncooperative.
