Identify a client who meets the criteria below for this nutrition assessment case study. • Be with at least one underlying health condition, from the list below:

1. Diabetes
2. Hyperlipidemia
3. Hypertension
4. Cardiovascular diseases
5. Post-stroke
6. Overweight/obesity issue
7. Undernourishment

• Is a community-dwelling individual, i.e. not in a nursing home or hospitalized.
• Mayor may not be on long-term medications or oral supplements.
• You are able to contact/meet regularly, be it face-to-face or through online communication platforms.
