Introduce the actual problem approved by your mentor and faculty. Paint a word-picture of the “presenting problem” or “quality or safety gap.” Write a concise and focused problem statement. Explain why the current system or process needs improvement. What are the causes of the problem? Can the causes be prioritized? List any other issues related to the problem. Provide detailed evidence that the problem exists. This can be found in the literature or national data-bases (evidence-based approach), and/or you can include agency or unit data related to the identified problem. What attempts have been made to solve the problem? Write a conclusion that summarizes the information presented.
