NURS 6720 Practicum Experience Journal
Journal Entries
• Describe a problem, issue, or situation that you have observed during your Practicum Experience (no more than a half page) (10 points)
The problem I have identified in the practicum experience is fear among the LGBT people to declare their sexual orientation. I am worried that minority groups cannot freely access quality and safe care in a friendly environment due to discrimination. One of the patients I interacted with disclosed that she had problems discussing her sexual orientation with healthcare workers. The patient disclosed that her sexual orientation always put her at risk of negative perception. LGBT people are at risk of mental health illnesses due to discrimination. Lack of access to quality and safe care puts them at risk depression and substance abuse. It is important to train the staff to handle patients from diverse backgrounds and with unique characteristics.
• Using no fewer than three peer-reviewed sources of evidence, analyze what you have observed within the context of your specialty using appropriate concepts, principles, and theories, giving special attention to observed events that vary from scholarly literature. (30 points)
One of the studies by Guest and Weinstein (2020) examines the role of anti-discriminatory practices on the level of trust and adherence to treatment. According to the findings, when patients realize that practitioners are appreciating their sexual orientation, they are likely to adhere to the treatment. Adherence and trust are important factors in the treatment of mental health illness and substance abuse disorders. Additionally, Radix and Maingi (2018) examine the role of cultural competence among healthcare workers in the delivery of safe and adequate care. The study recommends that training the staff on cultural competence is critical in realizing positive results in delivery of care in a diverse population. The findings caution against discrimination as a factor that can undermine quality care. Kcomt (2019) indicates that discrimination of transgender people in the American healthcare system is widespread. The effects of discrimination affect the willingness to attend treatment programs leading to adverse health outcomes.
• Explain how the problem, situation, or issue was handled in a manner that is consistent and/or a manner that is inconsistent with the theory, concepts, and principles detailed in the evidence. (30 points)
One of the approaches by the healthcare facility to address the crisis is training the staff on cultural competency. A training on cultural competence is in consistent with the theory and principles outlined in the evidence-based studies. The training improve respect, trust, and safety of patients in the healthcare environment. Advancing the skills of the staff creates an awareness of the diverse beliefs and traditions. It promotes inclusivity in the healthcare environment and improves adherence to treatment plans. The healthcare facility has been carrying out training on a quarterly basis to boost the cultural competence of the nurses.
• Given the various evidence-based approaches that can be used in handling the problem, situation, or issue, formulate a plan for approaching the matter differently. (30 points)
One of the different strategies that can be used to address the crisis in the healthcare environment is to diversify the workforce. Employing healthcare workers who resonate with LGBT can enhance the quality of the healthcare services. The organization should develop a policy to invite transgender nurses to apply for various positions in the healthcare facility. Healthcare facilities can reduce the risk of discrimination by integrating a diverse workforce. For instance, the diverse workforce will advocate for the rights of the LGBT people. Another method of approaching the issue differently is to encourage the patients to speak up or share opinions about the quality of the healthcare services. The facility can introduce suggestion boxes to help them listen to the concerns of the patients. The suggestions can attract confidential information of how patients have been treated. The information will help the facility to take necessary interventions to address the crisis.
